  1. What does your service do?

provision for children under 5 including those with disabilities and their families – 1:1, group support (universal and targeted) daycare/pre-school.

 2. Where is it located and what areas does it cover?

 See website: we have 2 centres in Blyth, 2 in Cramlington and 1 in Seaton Valley. We also use venues in the community e.g., libraries, community centres

3. Who does your service provide for?

 Parents to be and families with children under 5 years who live in Blyth Valley locality

4. How can I start using the service?

 You can make a self referral for services or can be referred by another professional working with you

 Many of our activities are free but there may be charges for activities arranged in partnership with other organisations

 for day care - workless families and those in receipt of DLA will qualify for funded 2 year old entitlement places. Day care provision may also be funded through children’s services disability team

 5. How are decisions made about who can use your service?

 Many of our services are universal access. For targeted services a diagnosis is required or information from a health professional confirming a likely diagnosis

  6. How do you communicate with service users and how are they

involved in decision making/planning?

 Leaflets and information about our groups and activities are available from the centre as well as being publicised on our website.

Large print is available and we can facilitate specialist communication (PECs, Makaton, signing)We work with other partners and translation services to reach

We consult with service users through questionnaires, evaluation of groups and activities, and parents and carers are represented on our decision making groups. children are involved through our children’s participation framework  .Our specialist provision at Blyth West, colgate cottage has been developed through a working group including representation from parents and carers and seeking the views of children with SEND

 We have open communication with parents and carers so that you are informed about how well my child or young person is doing? Regular reviews are held with parents

Parent training is available:  in the past year we have provided training on Behaviour and relationships, Including children with autism, Stress management; speakers from oral health promotions team, active Northumberland, National autistic society, Cerebra, Tim Lamb Children’s centre;

 7. Is your service fully accessible?

 All our buildings are fully wheelchair accessible? Loop system at our centre in seaton valley. We have disabled changing and

toilet facilities

Our centres, particularly the waiting areas are child friendly with plenty to engage children and we increase staff ratios where we are aware of children’s additional needs. Parents are supported in accessing the services with their children.

SEND had or are having?

in our daycare SENCOs have undergone training and provide support to colleagues. We have lead workers across the programme with lead in work with SEND
